| Job Title: | Utilization Review Case Manager |
| Job Duties: | Responsible for the Discharge Planning Program inclusive of all hospital units to ensure that the DC policies and procedures are implemented and followed accordingly. Evaluates patients medical records for accurate and complete documentation of medical necessity, treatment modalities and appropriate level of care. Determine medical necessity of hospitalization using evidence based criteria. Identifies planning needs of patients and coordinates all disciplines to ensure needs are met prior to discharge. Participate in discharge planning and transition of care. Participate in Hospital Quality Management Program. Participate in in site certifications to meet regulatory standards and serves as member of monitoring team in periodic program reviews. Facilitate the Discharge Planning meetings with the Quality Improvement Organization (QMO) in collaboration with the home health agencies. Reviews continued stay appropriateness on inpatient records concurrently during patients hospital stay. Reviews inpatient medical records for discharge. Durable Medical Equipment is arranged/prepared prior to discharge to ensure continuity of care. Communicate to Utilization Review Coordinator any discrepancies in documentation of services. Write statistical and narrative reports to Utilization Review Coordinator on outcomes of audits of provider services, highlighting areas such as timeliness of provider denials, certified admissions and extensions, and administrative stays. Refer clients to appropriate local quality care resources. Use or operate an electronic health record system. Maintain a log of chart reviews and other tracking mechanism. In the absence of the Utilization Review Coordinator, must perform duties and responsibilities as delegated. Performs other related duties as assigned by URC and of Director of CQPM. |

| Qualification Requirements: | Must hold current CNMI License as Registered Nurse (NCLEX-RN) and BLS required. Process strong communication, customer-service, problem-solving and analytical skills. Familiarity in electronic health records system. Ability to develop automated high-level care plans to provide each patient with the "right care at the right time" and work closely and effectively with physicians, hospital staff, co-workers and patients/families or individuals from culturally and linguistically diverse backgrounds. Able to work independently with minimal supervision. Computer literate. |
