| Job Title: | Medical Social Worker Assistant |
| Job Duties: | Works with the Medical Social Worker and Nursing Case Manager to: 1.Assist in performing social services assessment, counseling, education, and ongoing evaluation of clients who are receiving services under a medically approved Plan of Care. a.Complete assessments in a timely manner and in accordance with agency policy. b.Develop a written plan to provide information and direction to other health team members. c.Document all findings, plans, interventions, and client progress in the clinical record. d.Communicate plans and changes to the physician and/or nursing supervisor and other agency staff through the care plan, written progress notes, and participation in care conferences. 2.Prepare social/family histories to augment existing service, or as a guide in determining or changing level of service, taking into consideration. a.Family relationships. b.Meaning of illness to the client and his/her family. c.Cultural attitudes in relationship to illness. d.Meaning of “home” to the client. e.Ability of client and family to adjust to the care of the client in the home. f.The financial impact of client’s illness upon the family. 3.Demonstrate teamwork and effective communication to accomplish client, team, and agency goals. a.Participate in care conferences and other team or agency meetings. Shares areas of expertise and initiates interdisciplinary referrals. b.Educate team members and clients/families about community resources and how to access them appropriately. 4.Assist in the development, implementation, and evaluation of the ongoing quality improvement process. 5.Perform other related duties and responsibilities as deemed necessary. |

| Qualification Requirements: | 1.BA/BS Social Work, Psychology, Sociology, Counseling or related field 2.Minimum of two (2) years of experience in a health care setting or equivalent experience. Home care experience preferred. 3.CPR certification. 4.Excellent written and verbal communication skills and strong interpersonal skills. 5.Knowledgeable of resources available in the community. 6.Current driver’s license, good driving records and reliable transportation. |
