| Job Title: | Intake Coordinator |
| Job Duties: | The Intake Coordinator performs a variety of duties both clinical and clerical in nature. This position requires skill in planning, organizing and coordinating the delivery of patient care by all clinical staff personnel as well as the coordination of day to day patient care and office duties as assigned. The Intake Coordinator is primarily responsible for working with the Quality Assurance & Improvement Team, and the Referral Office Team to ensure proper initial coordination of care and services for all referrals. This position is to initiate and complete the OASIS Start of Care (SOC) for a home care referrals, initial Plan of Care (POC) to include any additional services and to verbally communicate with the referring and/or primary care physician regarding these services. The Intake Coordinator is also responsible for the coding and the care of the initial episode. This position requires skill in planning, organizing and coordinating the delivery of patient care by all staff personnel as well as the coordination of day to day office activities. |

| Qualification Requirements: | Graduate of an accredited school ofprofessional nursing, BSN, and PHS Certification preferred; and NCLEX Passer Current license as a Registered Nurse in the state(s) of practice. Minimum of seven (7) years nursing experience in an acute care hospital setting or equivalent experience with two (2) years of supervisory experience or equivalent. Excellent verbal and written communication skills and strong interpersonal skills. Knowledge of home health care federal and state regulations; strong understanding and adherence to quality assurance and improvement policies, procedures, criteria and regulations. Knowledge of reimbursement sources and documentation requirements within home health care. Excellent clinical documentation experience and direct patient care. |
